甘氨双唑钠在原发性肝癌经导管动脉化疗栓塞术中的应用观察

The clinical observation of sodium glycididazole in transcatheter arterial chemoembolization(TACE)

目的观察甘氨双唑钠(希美钠)联合经导管动脉化疗栓塞术治疗原发性肝癌的临床疗效。方法将116例原发性肝癌随机分成常规组(58例)和希美钠组(58例),常规介入治疗组用氟尿苷1.0g,艾达生40mg,顺铂80mg,碘化油8～10ml;希美钠组在常规介入治疗基础上加用希美钠0.75～1g。分别在术后1、3、6个月复查CT或MRI,肝功能,体重,甲胎蛋白等指标,以判断疗效。结果常规组CR11例,PR32例,SD12例,PD3例,有效率74.1%;希美钠组CR14例,PR38例,SD5例,PD1例,有效率89.7%。两组治疗有效率差异有统计学意义(P<0.05)。肝功能、副反应两组无差别,病人生存质量和体重均明显改善。结论TACE术联合希美钠可以显著提高原发性肝癌的治疗效果,并能够显著降低肝癌细胞的耐药性。

Objective To observe the effect of sodium glycididazole in TACE treating 58 HCC patients. Methods 116 patients were divided into CM-Na and control group randomly. In control group floxuridine 1.0 g, pharmorubicin 40 mg and C-DDP 80 mg were injected into the feeding artery of HCC through the catheter, and the trunk of the feeding artery was embolized with iodized oil 8 - 10 ml. In CM-Na group sodi- um glycididazole 0.75 - 1 g was used together with the drugs mentioned in control group. The result of treatments at the month 1,3 and 6 after the operation were evaluated by CT, MRl,weight, the liver function and AFP,etc. Results There were CR 11, PR 32, SD 12, and PD 3 cases in control group. The effect rate of the treatment arrived at 74. 1% , while in CM-Na group there were CR 11, PR 38, SD 5, and PD 1 case ,with an effect rate of 89.7%. The statistic analysis result showed that the difference of these two groups was obviously, （ P 〈 0.05 ）. There are no evi- dent difference in liver function and side-effects. However, the healthy and weight of the patients has been improved significantly. Conclusion The combination treatment of TACE and sodium glycididazole not only enhances the effect of treatment in HCC patients, but also greatly decreases the tolerance of drugs to hepatoma carcinoma cell.
