BiPAP呼吸机治疗COPD合并II型呼吸衰竭的临床价值

The Clinical Value of Noninvasive Bi level Positive Airway Pressure Ventilation in COPD Patients with Type-II Respiratory Failure

目的探讨双水平气道正压无创通气对慢性阻塞性肺疾病合并II型呼吸衰竭的治疗价值。方法将60例慢性阻塞性肺疾病合并II型呼吸衰竭患者随机分为BiPAP呼吸机治疗组(治疗组,30例)和药物治疗组(对照组,30例),比较动脉血气中pH、PaO2、PaCO2值及临床症状变化情况。结果治疗组治疗后2h、24h动脉血气中pH值和PaO2值逐渐上升,PaCO2值下降,临床症状明显改善。治疗组有效率为80.0%,高于对照组53.3%(P<0.05)。对照组动脉血气中pH、PaO2、PaCO2值及临床表现虽有不同程度的改善,但与治疗组同期比较,均有显著性差异(P<0.05)。结论无创双水平气道正压机械通气辅助治疗COPD合并II型呼吸衰竭具有肯定的临床疗效。

Objective To examine the effects of non-invasive bi level positive airway pressure ventilation in COPD patients with type,Ⅱ respiratory failure, Methods 60 cases of COPD with type-Ⅱ respiratory failure were randomly divided into two groups： mechanical ventilation group （treatment group） and medication group （control group）. The changes ofpH ,PaO2 and PaCO2 in artery sap and clinical condition were compared. Resuits In arteny sap pH and PaO2 elevated gradually, PaCO2 were reduced and clinical symptoms were improved prominently after 2h, 24h treatment of the treatment group; The effective rate of the treatment group was 81.81% which was higher than the control group （53.3%, P 〈0.05）;The control group, three parameters of arterial blood and clinical symptoms were also improved to a certain degree but was significant difference compared with the treatment group （P 〈0.05）. Conclusion Non-invasive mechanical ventilation is definitely valuable for COPD patients with respiratory failure.
