听觉诱发电位指数和脑电双频指数用于监测麻醉镇静深度的价值探讨 被引量:1

摘要 目的评价听觉诱发电位指数(AAI)和脑电双频指数(BIS)用于监测镇静深度的价值。方法40例腰-硬联合麻醉下行下肢手术患者,ASAⅠ~Ⅱ级。腰麻平面稳定后,丙泊酚效应室靶控输注清醒镇静,靶浓度从0.5μg/ml开始,输注5min后递增,递增梯度为0.5μg/ml,直至改良清醒镇静评分(OAA/S)为0分。记录预测效应部位浓度值每变化0.1μg/ml时的BIS、AAI和OAA/S评分,并记录脑电信号采集成功率和脑电信号的恢复时间。采用Spearman等级相关分析对AAI、BIS与改良OAA/S评分、EC进行相关分析。计算AAI、BIS预测不同改良OAA/S评分的预测概率(Pk)。结果异丙酚镇静时AAI和BIS与改良OAA/S镇静评级均有显著相关性(P<0.01);AAI的相关系数高于BIS(P<0.05),AAI和BIS对不同改良OAA/S评分时意识状态Pk值分别为0.93、0.92(P>0.05);异丙酚镇静时AAI、BIS与EC均有显著相关性(P<0.01);脑电信号采集成功率:BIS组>AAI组;脑电信号的恢复时间:BIS组>AAI组。结论丙泊酚TCI镇静时AAI同BIS一样能够准确反映患者的镇静深度变化,但AAI监测镇静深度的价值略高于BIS,AAI较BIS更能实时、快速监测麻醉镇静深度,BIS较AAI更容易获得良好的脑电信号。
