泪腺良性混合瘤的CT和MRI诊断

摘要 目的探讨CT和MRI对泪腺良性混合瘤的诊断价值。资料与方法回顾性分析45例经病理证实的泪腺良性混合瘤的临床和影像学资料,30例行CT检查,35例行MRI检查。结果45例中,起源于泪腺眶部43例,泪腺睑部2例。肿瘤呈椭圆形29例,圆形10例,不规则形6例;边缘光滑30例,边缘不规则呈分叶状或结节状15例。CT表现为等密度,大多数密度均匀。眶壁骨质受压、凹陷或缺损22例,骨质破坏伴眶壁硬化、肥厚6例,骨质增生硬化14例。MRT1WI上呈低信号25例,等信号10例,T2WI上呈高信号21例,等信号14例,增强后肿瘤呈中度至明显强化,大多数强化均匀。结论泪腺良性混合瘤的CT和MRI表现有一定特征,CT和MRI结合可以提高病变诊断的准确性。 Objective To discuss the value of CT and MR in diagnosing benign mixed tumor of the lacrimal gland. Materials and Methods The clinical data and imaging manifestations of 45 eases with benign mixed lacrimal gland tumor confirmed pathologicaLly were analyzed retrospectively, 30 eases were performed with CT scan, 35 eases with MRI scan . Results The lesions originated in the orbital lobe of the lacrimal gland in 43 eases and the palpebral lobe in 2 eases. The tumors showed oval shape in 29 cases, round shape in 10 eases, irregular shape in 6 eases, and had regular smooth margins in 30 eases, irregular lobulated or nodular margins in 15 eases. Most eases showed isodensity on CT. Orbital bone compression or defect was seen in 22 eases, bone destruction with hyperostosis in 6 eases, bone sclerosis in 14 eases. MR T1 - weighted imaging demonstrated hypointense signal in 25 cases and isointense signal in 10 eases. MR T2 - Weighted imaging demonstrated hyperintense signal in 21 eases, isointense signal in 14 eases. Postcontrast MR imaging showed moderate to markedly enhancement. Conclusion Benign mixed tumor in the lacrimal gland have some imaging features, combined with CT and MRI could improve the diagnostic accuracy.
