7.5%氯化钠对重度创伤失血性休克病人麻醉诱导时血流动力学的影响

摘要 目的探讨小容量7.5%氯化钠溶液对重度创伤失血性休克病人全麻诱导时血流动力学改变的影响。方法80例创伤性休克病人随机分为生理盐水组(Ⅰ组)和7.5%氯化钠溶液组(Ⅱ组),每组40例。全麻诱导前5 min,Ⅰ组给生理盐水8.0 mL/kg快速静脉滴注,Ⅱ组静注7.5%氯化钠溶液1.0 mL/kg。诱导时依次静注咪唑安定0.1 mg/kg、芬太尼2.0μg/kg、阿曲库胺0.3 mg/kg,同时连续监测ECG、HR、BP、CVP。结果基础值比较,在各时点Ⅰ组HR、SBP显著下降(P<0.05);而Ⅱ组HR显著增快(P<0.05),SBP差异无显著意义,两组间的BP、CVP无显著差异。结论7.5%氯化钠溶液有利于重度创伤失血性休克病人全麻诱导时血流动力学的稳定。 Objective To discuss the affection during anesthesia inducing for severe tramnatic hypovolemic shock patient with 7.5 % sodium chloride. Methods 80 cases of such patient were divided into two groups control group (NS) (who was given saline 8 mL/kg )and observed group who was given 7.5 % sodium chloride. The ECG, HR, BP and CVP were observed while midazolam, fentanyl and other drug were given. Results There was no significant difference between two groups in SBP, BP and CVP. The HR in group A was decreased and in group B increased. Conclusion The 7.5 % sodium chloride might benefit the hemodynamic stable during anesthesia inducing for such patient.
