异己手综合征(附2例临床分析)

Alien hand syndrome.

本文报道２例异己手综合征（ＡＨＳ）典型病例，病因分别为脑梗塞和皮质基底节变性，临床均有该病特征性的双手协同障碍即双手拖拉战，此外尚有运动滞留、自责、皮质反射性肌阵挛及镜像书写等附加症状。ＣＴ及ＭＲＩ显示病变累及额叶前内侧和胼胝体。经对症治疗两例均有好转。本文并就ＡＨＳ的临床表现、诊断及可能的解剖学基础进行探讨。

Two typical cases with alien hand syndrome(AHS) are described,The causes were cerebral infarction and corticbasal ganglionic degeneration (CBD). All two cases showed bimanual coordination, eg.tug of war and motor preservation, autocriticism,cortical reflex myoclonus and mirror writing as well. CT and MRI showed a lesion involving mesial frontal region and fibers of the corpus callosum.Symptoms of all two cases were improved after expectant treatment.The clinical characteristics,diagnosis and possible anatomical bases of AHS were discussed.
