老年人髋臼骨折的手术治疗

目的通过对17例老年人髋臼骨折切开复位内固定的手术治疗,探讨处理此类损伤应注意的几个问题。方法依据AO分类:A2型:2例,A3型:2例,B1型:3例,B2型:4例,B3型:3例,C1型:3例,均采用切开复位内固定手术治疗。结果随访6～48个月,优者12例,良好者3例,差者2例,手术优良率达90%。结论对所有的老年患者进行认真的手术评估。综合分析病情后在病情允许时应尽早考虑手术、明确骨折分型、选择适合入路、骨折的解剖复位牢靠固定、软组织的稳固缝合、早期的康复锻炼是获得良好疗效的关键。

Objective： Through fixation for acetabular fracture the operative treatment of of 17 old persons to discuss open reduction and internal several problems on dealing with this kind of injury. Method： Based on the classification of AO： A2 type： 2 cases, A3 type： 2 cases, BI type： 3 cases, B2 type： 4 cases, B3 type： 3 cases, CI type： 3 cases, all of the above have been taken operative treatment of open reduction and internal fixation. Result： Through 6-48 months follow-up care, 12 fine cases, 3 good cases, 2 poor cases, the fine rate of the operation reached 90%. Conclusion： Careful operation evaluation must be taken to all of the old person patients. The keys to a fine curative effect are： considering to do the operation as soon as possible after integrate analysis of the injury if the patient＇s condition permits~ to clarify the type of the fracture; to choose the suitable entry way, the anatomical reduction and firm fixation for the fracture; the stable suture for the cartilaginous tissue, and the recovery practice during the earlier period.
