摘要
目的观察醒脑静注射液治疗急性心肌梗死(AMI)的临床疗效及其对左室射血分数(LVEF)、肌酸激酶同工酶(CK-MB)、C-反应蛋白(CRP)、D-二聚体(DD)、纤维蛋白原(Fib)的影响。方法76例AMI患者随机分为对照组(n=37)和醒脑静组(n=39)。对照组采用常规治疗(应用硝酸酯类药物、阿司匹林、β-受体阻滞剂,溶栓及经皮冠状动脉介入治疗等);醒脑静组在常规治疗的基础上静脉滴注醒脑静注射液20mL/d,静点10~14d。分别观察两组治疗前后胸痛发作次数、心电图的变化及LVEF、CK-MB、DD、Fib、CRP的变化。结果醒脑静组临床治疗总有效率显著高于对照组(P<0·05);醒脑静组CRP及CK-MB峰值水平均显著低于对照组(P<0·01);醒脑静组LVEF升高程度及DD、Fib降低程度较对照组更为显著(P<0·05)。结论醒脑静注射液治疗AMI有良好的临床疗效,可有效改善患者心功能,降低患者血浆CK-MB、DD、Fib和CRP的水平。
Objective To observe the curative effect of the Xingnaojing injection for acute myocardial infarction(AMI) and to study the effect of Xingnaojing injection on left ventricular ejection fraction (LVEF), creatine kinase isoenzyme MB ( CK - MB ), D - Dimer ( DD ), fibrinogen (Fib), C - reactive protein (CRP) in the patients with AMI. Method 76 patients with AMI were randomly divided into Xingnaojing group ( n = 39) and control group ( n = 37 ). Conventional therapy ( nitrate, β - blocker, aspirin, thrombolysis, PCI etc. )were used in the control group; 20 mL/d Xingnaojing injection iv gtt were given to the Xingnaojing group for 10 - 14 days, besides the conventional therapy. The clinical symptoms, ECG and LVEF, CK -MB, DD, Fib, CRP of the two groups were observed. Result The total effective rates were significantly higher in Xingnaojing group than in control group ( P 〈 0.05 ). The serum CRP and CK - MB peak levels of Xingnaojing group were obviously lower than those of control group ( P 〈 0. 01 ). LVEF increased and serum levels of DD, Fib decreased after the treatment in two groups, but the changes was more obvious in Xingnaojing group than in control group (P 〈 0.05 ). Conclusion The better curative effect of the Xingnaojing injection for the treatment of AMI was verified, Xingnaojing injection can effectively improve LVEF and reduce the serum levels of CK - MB, DD, Fib, and CRP in the patients with AMI.
