肺炎婴幼儿鼻咽分泌物呼吸道病毒、肺炎衣原体的检测分析

摘要 【目的】分析温州地区肺炎婴幼儿鼻咽分泌物7种呼吸道病毒、肺炎衣原体的感染及混合感染情况。【方法】选择2004年11月~2006年11月因肺炎而在温州医学院附属育英儿童医院住院治疗的婴幼儿,取其鼻咽分泌物做直接免疫荧光检测筛查7种呼吸道病毒与肺炎衣原体抗原,分析其结果。【结果】460例患儿送检标本,阳性260例(56.5%),其中合胞病毒(RSV)182例(39.6%)、腺病毒(ADV)2例(0.4%)、流感病毒A型(IVA)4例(0.9%)、流感病毒B型(IVB)2例(0.4%)、副流感病毒I型(PIVI)7例(1.5%)、副流感病毒III(PIVIII)16例(3.5%)、肺炎衣原体(CPn)42例(9.1%)。混合感染5例(1.1%),其中RSV+IVA2例(0.4%),RSV+CPn3例(0.7%)。RSV冬春两季高发,PIVI以秋季为主,PIVIII与CPn则无明显季节性。【结论】RSV是目前引起婴幼儿肺炎的主要病毒,CPn次之。RSV在温州地区的流行见于冬春季。CPn感染四季散发,存与RSV混和感染情况,在临床诊治中应给予关注。 [Objective] To investigate the status of seven different respirovirus and ehlamydia pneumoniae(CPn) infection and mixed infection in infant pneumonia in Wenzhou. [Methods] Patients suffered from pneumonia,admitted to the Affiliated Yuying Children's Hospital of Wenzhou Medical College from November, 2004 to November,2006 were chosen. Their nasopharyngeal sectetions (NPS) were obtained for identification of seven different respirovirus and CPn antigens by direetImmunofluoreseenee (DIF)assay. [Results] Among all the 460 samples,260(56.5%) positive results were determined. 182(39.6%) positive sample were respiratory syneytial virus (RSV),2(0. 4%) were adenovirnses(ADV),4(0. 9%) were influenza virus A(IVA),2(0.4%) were influenza virus B( IV B), 7 (1.5 % ) were parainfluenza virus I(PIVI), 16 (3. 5 % ) were parainfluenza virus III(PIVm) and 42 (9. 1%) samples were positive of ehlamydia pneumoniae(CPn). Also there were 2(0. 4%) eases, whieh were positive of RSV and IVA together, and 3(0.7 % ) were RSV and CPn mixed infection. RSV positive rate was high in winter and spring and PIVI was detected mostly in autumn,while the distribution of PIVIII and CPn had no evident seasonality. [Conclusions] RSV is one of the main factors causing of pneumonia in infants,folowed by CPn. RSV is prevalent both in spring and winter in Wenzhou area. Doctors should pay enough attention in clinical work, because CPn is not only detected in all seasons, but also has mixed infection with RSV.
