摘要
目的:探讨ATP敏感性钾通道(KATP通道)开放剂尼可地尔对离体大鼠缺血后心功能及合用Na+/H+交换阻滞剂Cariporide对心肌梗死范围的影响。方法:用Langendorff装置,观察不同浓度尼可地尔对冠状动脉(简称冠脉)灌流量及缺血后心功能的影响。应用离体大鼠双冠脉分别灌流模型研究心肌梗死面积,将左冠脉灌流量减到原灌流量的5%,行1h缺血及2h再灌注。结果:尼可地尔1μmol/L可明显增加冠脉灌流量,20μmol/L时左室收缩压明显下降,在缺血前应用1μmol/L尼可地尔有改善缺血后心功能的作用,对照组经1h缺血及2h再灌注后心肌梗死范围为(47.2±9.0)%,尼可地尔和Cariporide在缺血中应用缩小心肌梗死范围分别为(26.5±7.6)%和(30.7±6.6)%,两者合用时心肌梗死范围为(19.9±5.3)%。结论:尼可地尔有明显扩张冠脉、保护缺血后心功能及缩小心肌梗死范围的作用。联合应用Cariporide可进一步缩小心肌梗死范围。
Objective:To study the effects of ATP-sensitive K^+ channel (KATP channel) opener,Nicorandil,on the function of isolated ischemic rat heart,and its effect on myocardial infarct size when it is used together with Na^+/H^+ exchanging inhibitor,Cariporide. Methods:The effects of different concentrations of Nicorandil on coronary blood flow and post-ischemic cardiac function were studied by the Langendorff apparatus. The rat model in whom both coronary arteries were infused separately was used to observe the myocardial infarct size. After the left coronary artery blood flow was reduced to 5% of the basal flow,1-hour ischemia and then 2-hour reperfusion were performed. Results:The coronary artery blood flow increased at the presence of 1 μmol/L Nicorandil,and the left ventricular systolic pressure decreased significantly when the concentration reached 20 μmol/L. The administration of 1 μmol/L Nicorandiol before ischemia could improve post-ischemic cardiac function. After 1-hour ischemia and then 2-hour reperfusion,the myocardial infarct size was 47.2%±9.0% in control group,26.5%±7.6% in Nicorandiol group,30.7%±6.6% in Cariporide group,and 19.9%±5.3% in combination of Nicorandiol and Cariporide group. Conclusion:Nicorandil could expand the coronary artery obviously,protect the post-ischemic cardiac function,and reduce the myocardial infarct size. The combination of Nicorandil and Cariporide may further reduce the myocardial infarct size.
