复方苦参注射液辅助双介入治疗原发性肝癌疗效观察

Therapeutic effects of complex kushen injection combined with double interventional therapy on treating primary hepatocarcinoma

[目的]评价复方苦参注射液辅助双介入(肝动脉化疗栓塞加无水乙醇注射)治疗原发性肝癌的疗效及不良反应。[方法]对86例原发性肝癌患者进行连续3周期肝动脉化疗栓塞,化疗方案用FAM方案,并加用3周期无水乙醇肿瘤内注射,治疗组(46例)同时静脉点滴复方苦参注射液。对照组(40例)常规护肝治疗,并比较2组治疗前后肿瘤大小、Karnofsky评分、中医证候、T细胞亚群和不良反应情况。[结果]治疗组远期生存率高于对照组(P<0.05);Karnofsky评分、临床症状缓解率优于对照组(P<0.05),CD3+、CD4+、CD8+、CD4+/CD8+比值、NK细胞活性,2组比效差异有统计学意义(P<0.05),治疗组不良反应显著低于对照组。[结论]复方苦参注射液辅助双介入治疗原发性肝癌可提高远期生存率,调节免疫水平,改善生活质量,减轻不良反应。

[Objective]To evaluate clinical effect and side effect of complex kushen injection combined with double intervention on primary hepatocarcinoma . [Methods] Eighty six patients with hepatocarcinoma were accepted arterial embolismic chemotherapy with FAM program for three therapeutical cycles and percutaneous ethanol injection for three therapeutic cycles. The forty six patients in the treatment group were injected complex kushen injection for adjunctive therapyand other forty patients in control group were given conventional liver protectant. The tumor size, karnofsky score, clinical symptoms, NK cell, T cell-sub group and adverse effect before and after treatment between the two groups were compared respectively. [Results] Compared with the control group, the long-term survival rate, Karnofsky scores and the remission rate of clinical symptoms（65% vs 91. 3%）in the treatment group were higher（P〈0.05）. The activity ratio of CD^3＋, CD^4＋, CD^8＋ , CD^4＋/CD^8＋ and NK cells was significantly different between the two groups. There were no obvious adverse effect in the treatment group. [Conclusion ]Complex kushen injection combined with double intervention on primary hepatocarcinoma can effectively enhance and regulate the immunity of patient with tumor, relieve clinical symptoms, improve quality of life and has no adverse effects.
