钙拮抗剂对高血压并左室肥厚患者的血浆脑钠素的影响

The effect of calcium antagonist on plasma brain natriuretic peptide in treating hypertensive patients with left ventricular hypertrophy

目的:研究长效钙拮抗剂治疗高血压并左室肥厚对血浆脑钠素(BNP)的影响,为临床选择降压药提供更客观的依据。方法:①比较原发性高血压并左室肥厚、无左室肥厚及健康对照组BNP浓度的差别;②选择左室肥厚组作为随访对象,予以长效钙拮抗剂治疗(6±3)月,比较BNP、左室质量指数(LVMI)在治疗前后的变化。结果:①高血压并左室肥厚组血浆BNP水平显著高于高血压无左室肥厚组和健康对照组(P<0.001);②高血压患者血浆BNP与LVM I呈正相关关系(相关系数r=0.551,P<0.001);③使用长效钙拮抗剂治疗高血压并左室肥厚患者(6±3)个月,血浆BNP水平和LVM I均显著降低(P<0.001)。结论:长效钙拮抗剂治疗高血压并左室肥厚患者能显著降低患者BNP水平,能够逆转左室肥厚。

Aim： Investigating the effect of long acting calcium antagonists on the level of plasma brain natriuretic peptide （BNP） in treating the hypertensive patients with left ventricular hypertrophy（ LVH）, to provide lab evidence for clinical practice in choosing anti-hypertension drug. Methods： Compare the differences of the BNP level among the ＂LVH group＂, the ＂NON-LVH group＂ and healthy control; The patients with LVH were given long acting calcium antagonists for （6 ＋ 3） months, the differences in the BNP level and LVMI were evaluated before and after treatment. Results： （ 1 ） The level of plasma BNP significantly increased（P 〈0. 01） in hypertensive patients with LVH compared with those without LVH and healthy subjects. （2） The plasma BNP positively correlated with LVMI in hypertensive patients（ r = 0. 551, P = 0. 000）. （3）Both the level of plasma BNP and LVMI significantly decreased （P = 0. 000） in hypertensive patients with LVH treated by long-acting calcium antagonists after （6 ± 3 ） months. Conclusion：Both the level of plasma BNP and LVMI significantly decrease in hypertensive patients with LVH who are treated by long-acting calcium antagonists. Accordingly long-acting calcium antagonists can reverse LVH.
