摘要 目的:观察311nm窄谱中波紫外线(NB-UVB)对银屑病的疗效,以及治疗前后皮损角质形成细胞周期蛋白D1、E(cyclinD1、E)的表达。方法:以Cosmedico皮肤检测仪检测25例患者的最小红斑量(MED),以PASI评价疗效,并以免疫组化法检测皮损部位cyclinD1、E的表达。结果:NB-UVB治疗银屑病的临床有效率为92%;光疗前绝大多数皮损可检测到两种蛋白,但分布于不同的表皮细胞层,光疗后皮损中cyclin D1、E的表达较治疗前明显下降(均P<0.01);cyclin D1、E表达情况与PASI值呈正相关。结论:NB-UVB治疗银屑病安全、有效;下调表皮角质形成细胞中cyclin D1、E的表达可能是NB-UVB治疗银屑病的机制之一。 Objective: To investigate the efficacy of NB - UVB phototherapy in psoriasis vulgaris and to detemtine the expression of cyclin D1 and E before and after photothempy. Methods: A total of 25 patients with psoriasis vulgaris were treated by NB - UVB radiation twice a week. Minimal erythemal dose (MED) determination was performed by Cosmedico TL - 01 UVB source. The severity of the disease was assessed at baseline and endpoint by two independent investigators based on the standard Psoriasis Area and Severity Index (PASI) method was adopted to measure the expressions of both cyclins. Results: The clinical efficiency of NB- UVB phototherapy for psoriasis was 92%. The cyclin D1 and E were present in almost every psoriatic lesion before phototherapy. Moreover, these cell - cycle markers were distributed in all epithelial layers. The expression 9f cyclin D1 and E was decreased remarkably after irradiations ( P 〈 0.01) and was positively correlated with PASL Conclusion: NB - UVB therapy is safe and effective for psoriasis vulgaris. Both cyclin D1 and E seem to be reduced in psoriasis. The mechanism of NB- UVB in the treatment of psoriasis might be related to the inhibition of cyclin D1 and E expression.
