高频电刀对胆囊切除术切口脂肪液化影响的临床研究

Clinical study on influence of high frequency electric surgical knives on abdominal incision fat liquefaction of cholecystectomy

目的:探讨高频电刀对胆囊切除术切口脂肪液化的影响。方法:用手术刀切开皮肤,电刀切开皮下脂肪层、腱膜和肌层(实验组,n=232)。用手术刀切开皮肤、皮下脂肪层,电刀切开腱膜及肌层(对照组,n=240)。观察两组病人术后切口愈合情况。结果:实验组有19例(8.2%),对照组有9例(3.8%)切口皮下脂肪液化。实验组与对照组的差异有高度显著性意义(x2=4.167,P<0.05)。结论:高频电刀可使胆囊切除手术切口皮下脂肪液化率明显增加,愈合延迟。

Objective To explore the influence of high-frequency electric surgical knives on abdominal incision fat ciquification of cholecystectomy. Methods cases underwent cholecystetomy by using lancet cutting skin, and electilc surgical knife cutting subcutaneous fat, muscle layer and pritendineum（EK group）, 240 cases by only using com- mon lancet cutting all above tissues （ML group） observed the healing condition.Results 19 cases of subcutaneous fat liquification （EK group） were observed ,compared with only 9 cases that in ML group ,there were significant differences between two groups （x^2=4.167,P〈0.05）. Conclusions High-frequency electric knife using in cholecystetomy may lead to the increase of postoperative subcutaneous fat liquification,and delay abdominal incision healing.
