输尿管镜气压弹道碎石治疗输尿管中下段结石(附52例报告)

Transureteroscopic pneumatic lithotripsy in treatment of the middle and lower ureteral calculi(report of 52 cases)

目的探讨输尿管镜气压弹道碎石术治疗输尿管结石的临床疗效。方法自2005年5月～2007年4月,采用输尿管镜下气压弹道碎石术治疗输尿管中下段结石52例。结果一次性结石粉碎率为86%(45/52),并发症为9.6%(5/52),其中输尿管穿孔2例,结石上移2例,尿路感染1例。结论输尿管气压弹道碎石术创伤小,安全性高,疗效确切,可作为输尿管中下段结石的首选治疗方法,如与体外冲击波碎石术(ex-tracorporeal shock wave lithotripsy,ESWL)等体外碎石方法结合应用疗效更佳。

[Objective] To explore the clinical effects of ureteroscope pneumatic lithotripsy for the treatment of ureteral calculi. [Methods] From May 2005 to April 2007, 52 patients with stone in ureter were treated with the above-mentiond lithotriptor. [Results] 86%（45/52） of stones were fragmented in one treatment session. Complications were observed in 9.6% of the cases （5/52） including 2 cases of ureteral perforation and 2 cases of calculi up-shift, and 1 case of urinary infection. [Conclusions] Transureteroscopic pneumatic lithotripsy is safe, effective and minimally invasive and probably the frist choice of treatment for the middle and lower ureteral stone. The result would be better if used combined with other extracorporeal lithotripsy such a extracorporeal shock wave lithotripsy （ESWL）.
