Hibbard护理模式干预对老年2型糖尿病患者血糖控制的作用

The effect of Hibbard's diabetes nurse model on blood sugar control in older patients with type II diabetes

目的评估临床护理干预对血糖不稳的老年Ⅱ型糖尿病患者血糖控制的效果。方法130例门诊老年Ⅱ型糖尿病患者按纳入标准随机分为干预组(n=65)和对照组(n=65),干预组患者在常规服用降血糖药物的同时,根据Hibbard护理模式并加以改进,通过护理培训、饮食指导等护理干预,而对照组采用常规药物治疗和家庭护理手段,通过电话随访监测患者心理和生化指标来评估护理干预的效果。结果经过护理干预后,患者对药物治疗的依从性显著提高,经过12周的随访,患者糖化血红蛋白、空腹血糖和餐后2 h血糖均显著低于随访前的基数值(P<0.01)。结论Hibbard护理模式可提高患者对药物治疗的依从性,突出个体化治疗特点,改善患者血糖控制效果。

Objective To evaluate the effect of a diabetes nurse clinical intervention in blood sugar control in older patients with type II diabetes whose blood sugar concentrations were unstable. Methods A total of 130 older patients with type II diabetes whose blood sugar concentrations were unstable in our out-patient department were randomly divided into 2 groups： control group（65cases） and intervention group（65 cases）.Patients in the intervention group were given regular hypoglycemic agent and nurse intervention such as nurse training,diet direction and so on according to Hibbard＇s diabetes nurse model.Patients in the control group were given regular hypoglycemic agent and family nurse.The effect of nurse intervention was evaluated by detection of biochemical data and monitoring of psychological index through telephone follow-up survey. Results After 12 week follow-up survey,patients in the intervention group were more obedient to treatment,and glycosylation hemoglobin,fasting blood-glucose concentration and blood-glucose concentration in 2 hrs after diet significantly lower than those before intervention（P〈0.01）. Conclusion Hibbard＇s diabetes nurse model can increase patients＇ obedience to treatment and improve the effect of blood-glucose control as well as stress individual treatment.
