摘要
目的评价多象限水分离技术治疗儿童先天性白内障的临床疗效。方法对51例(80只眼)儿童先天性白内障,用多象限水分离技术实施白内障超声乳化吸除,联合人工晶体植人术的患儿进行回顾性分析,记录术前及术后的裸眼视力,最佳矫正视力,观察手术并发症和术后眼部情况,随访时间为6~36月。结果术后最佳矫正视力≥0.5的为31.25%。随访期间62.5%的病例未出现视轴区后囊膜混浊,并发症为前房渗出物、虹膜后粘连等。结论多象限水分离技术是一个简便易行的手术方法,能有效清除晶体皮质及残留的晶体纤维,预防或延缓后发性白内障的发生。
Objective To evaluate the efficacy of multiquadrant cortical-cleaving hydrodissection in pediatric congenital cataract surgery. Methods 51 cases (80 eyes) received multiquadrant hydrodissection with phacoemucification of the posterior chamber combined with intraocular lens implantation. Preoperative and postoperative visual acuity and best correct visual acuity were recorded. Intraoperative and postoperative complications were observed. The follow-up period was from 6,-~36 months. Results The mean postoperative best corrected distant visual acuity reached 0.5 or above in 80 eyes (31.25%), and optic axis posterior capsular opacity was not found in 62.5% eyes. The major complications were anterior chamber fibrous exudation and iris posterior synechia. Conclusion Multiquadrant cortical-cleaving hydrodissection is a simple and inexpensive procedure and facilitates lens substance removal in pediatric congenital cataract surgery. It is a safe and effective way in preventing the posterior capsular opacity from the visual axis.
