经颅多谱勒超声监测小儿先心病脑血流的临床价值探讨 被引量:1

摘要 目的:探讨经颅多谱勒超声(TCD)监测小儿先天性心脏病脑血流的临床意义。方法:67例先心患儿分为三组:青紫组(12例),活动后青紫组(38例),无青紫组(17例);正常对照组1组(20例)。应用TCD仪,常规探查颅内双侧大脑前、中、后动脉和颈内动脉、椎动脉及基底动脉共11条动脉;探测参数有:脑动脉平均峰流速(Vm)、左右大脑半球血流速度对称性、频谱形态、血流声频、血管波动指数(PI);对异常的其中28例做动态TCD观察。结果:TCD异常表现为Vm增高或下降、左右半球血流速度不对称、涡流频谱、粗糙血流声频、PI下降等。青紫组12例均异常;活动后青紫组异常34例,正常4例;无青紫组异常2例,正常15例;正常对照组异常0例,正常20例。青紫组、活动后青紫组TCD异常率高于无青紫组和正常对照组(χ2=30.13,P<0.01),而前两者间及后两者间差异无显著性。χ2检验显示青紫组和活动后青紫组PI下降,Vm增高或下降、双半球血流速度差、涡流频谱、粗糙血流声频异常率均比活动后无青紫组和正常对照组高,P均<0.01;青紫组和活动后青紫组各项参数差异均无显著性(P>0.05)。结论:TCD简便、无创,有助于长期随访观察小儿先天性心脏病患儿病情及监测及其脑血管早期功能性变化,以指导临床治疗,为小儿先心外科择期手术创造最佳时机。
