摘要 目的:探讨颌骨骨折的不同影像学诊断价值。方法:对125例不同类型的上、下颌骨骨折的常规X线、常规横断位CT扫描和CT三维重建技术的显示情况与手术结果进行对照分析。结果:对于下颌骨体部、角部和升支骨折,三种影像学都能很好的显示;对于髁状突和喙突骨折,CT三维重建技术优于常规X线和常规横断位CT扫描;常规X线显示上颌骨骨折较差,而常规横断位CT扫描和CT三维重建技术显示的准确性比较接近。CT三维重建技术可以直观和立体的显示颌骨骨折线。结论:三种影像学检查各有优缺点,为提高骨折显示的准确率,根据骨折部位结合患者的经济条件,联合应用比较妥当。 Objective To explore the value of imaging diagnosis of the facial fractures. Methods One hundred and twenty-five cases of facial fractures were examined with the conventional X-ray plain film, conventional CT and 3D reconstruction before operation. The results of three methods were compared with the operation findings. Results Fractures in body, angle and ascending branch of mandible could be shown well by all these imaging methods. 3D reconstruction was better than conventional CT and conventional X-ray plain film in the diagnosis of fractures in condyle and coronoid process. Conventional X-ray plain film was poor in diagnosis of the maxillary fractures. Conventional CT and 3D reconstruction were similar in the diagnosis of the maxillary fractures. 3D reconstruction showed the jaws fractures directly and stereoscopicly. Conclusion Each of these three methods has its advantage and shortage in showing facial fractures. In order to raise the accurate rate of diagnosis, combination of three methods is necessary.
