摘要
目的:探讨严重创伤病人血清肿瘤坏死因子α及C-反应蛋白的变化与其预后的关系。方法:选择160例严重创伤病人动态监测入室当时12,24,48,72h血清TNF-α及CRP水平的变化。结果:严重创伤病人入室当时血清肿瘤坏死因子α及C-反应蛋白水平明显高于正常值;好转组24~48h后TNF-α及CRP均逐渐下降,而死亡组TNF-α及CRP于48h后仍明显高于好转组(P<0.05)。结论:严重创伤病人血清TNF-α及CRP水平与疾病的发生、发展密切相关,此两项指标的检测对判断其预后有重要意义。
Objective: To observe changes in serum tumor necrosis factor-α(TNF—α) and C-reactive protein(CRP) levels in patients with severe trauma, and their relation to the prognosis. Methods: The changes in serum TNF—α, CRP levels were observed on the time of injury, 12h, 48h and 72h after injury in 160 patients with severe trauma. Results: The expression of TNF—α and CRP in patients with severe trauma was obviously higher than that in normal controls; in the cases with post-therapeutic improvement 24h to 48h after injury, TNF—α and CRP levelsgot lower; but the cases with non-survivors was obviously higher than the cases with posttherapeutic improvement (P〈0.05.) Conclusions: It is suggested that the expression of TNF—α and CRP in severe trauma obviously increased and was closely related to the prognosis. TNF—α and CRP levels may serve as useful indexes in judging the progress and prognosis of patient with severe trauma.
