胸水ADA与LDH、CRP检测在鉴别恶性与结核性胸水中的作用 被引量:2

摘要 目的评价胸腔积液腺苷脱氨酶(ADA)、乳酸脱氢酶(LDH)、C-反应蛋白(CRP)在恶性胸水及结核性胸水中的诊断价值。方法对78例患者(恶性44例,结核性34例),每例均检测患者胸水ADA、LDH、CRP。结果恶性胸水组LDH水平显著高于结核组(P〈0.001),而其ADA、CRP显著低于结核组(P〈0.001)。胸水ADA、LDH和CRP联合检测,对恶性胸水判别正确率为97.7%结论LDH、ADA、CRP可作为良恶性胸水鉴别的参考指标。 Objective To evaluate the significance of ADA, LDH and CRP in the diagnosis of malignant pleural effusions and tuberculosis pleural effusions. Methods This study consistedof 78 patients diagnosed with malignant (44),tuberculous (34) pleural effusions. We examined the following biochemical parameters in the pleural fluid: adenosine deaminase, lactate dehydrogenase and C reactive Protein. Results Malignant pleural effusions had a higher level of LDH than tuberculous pleural effusions(P〈0.001). Tuberculous effusions had significantly higher levels of ADA and CRP than those of malignant effusions (P〈0.001).of the 44 malignant cases, 43(97.7%) were diagnosed correctly when ADA, LDH and CRP were evaluated together. Conclusions The changes of ADA, LDH and CRP in pleural effusions are of clinically diagnostic value in distinguishing tuberculosis from malignant pleural effusions.
