第二磨牙纳入矫治系统矫正成人深覆的临床研究

Clinical research of deepbite correction in adult patients by bringing the second molars into orthodontic system

目的:探讨将第二磨牙纳入矫治系统矫正成人深覆的临床效果。方法:对22例成人深覆患者常规采用MBT直丝弓矫治系统,并早期在第二磨牙上粘接颊面管,将摇椅弓延伸至第二磨牙,比较矫治前后X线头影测量值。结果:治疗后,在矢状方向上∠SNA、∠ANB、∠U1-NA、∠L1-NB均有减少,∠SNB有所增加(P<0.05)。在垂直方向上,前下面高和后面高分别平均増加3.6mm和0.9mm,面部下1/3比例增加;而覆、覆盖则分别平均减小3.4mm和2.7mm,有效地打开了深覆。所有患者治疗后磨牙均呈中性或完全远中关系,尖窝锁结关系良好,覆覆盖正常,牙及颌骨的不协调关系得到矫正,面型显著改善。结论:将第二磨牙纳入矫治系统来矫正成人深覆,有效地增加了前下面部垂直高度,是一种提高打开成人深覆效率的有效方法。

Objective： To investigate the effects of deepbite correction in adult patients by bringing the second molars into the orthodontic system. Methods： 22 cases were treated with MBT straight wire arch appliances, while the buccal tubes were bonded on the second molars at the early stage, and reverse curve of spee archwire was extended to the second molars. Cephalometric analysis was carried out before and after the treatment. Results,. The SNA, ANB, U1-NA and L1-NB angles were significantly decreased, while the SNB angle was increased significantly after treatment. Anterio-inferior facial height and posterior facial height were vertically increased by 3.6 mm and 0. 9 mm, and proportion of facial inferior height was increased as well. Overbite and overjet were greatly decreased by 3.4 mm and 2. 7 mm. The deepbite was corrected effectively. All patients had normal overbite and overjet with good intercuspid relationship after treatment. The discrepancy between maxilla and mandible was corrected and profiles of patients were improved obviously. Conclusion ： It is an effective approach of deepbite correction in adult patients to bring the second molar into the orthodontic system, and it may increase the anterio-inferior facial height as well.
