心尖肥厚型心肌病患者心电图的诊断意义

The diagnositic significance of electrocardiographic feature change in patients with apical hypertrophic cardiomyopathy

目的探讨心尖肥厚型心肌病患者的心电图特征性改变及临床诊断意义。方法分析38例心尖肥厚型心肌病患者的心电图及超声心动图资料。结果心尖肥厚型心肌病合并有心电图异常和超声心动图异常改变者38例(100%),其中伴胸前导联巨大倒置T波≥0.2mV～0.3mV,以V3～V4最明显29例(76%);左胸V4～V6导联R波电压增高,且RV4>RV5>RV6者26例(68.4%);ST段压低,以V3～V4最明显者29例(76%);超声心动图示心尖部肥厚达15mm或以上伴心尖部心腔狭小者38例。结论心电图对心尖肥厚型心肌病具有早期诊断价值和预测意义。

Objective Study electrocardiographic feature change of apical hypertrophic cardiomyopathy and its clinical diagnostic significance. Methods Analyse 38 patients of apical hypertrophic cardiomyopathy with their electrocardiogram and ultrasound cardiography （UCG） data. Result All of 38 patients had electrocardiogram abnormality and UCC abnormality （100%） compared with deep inversion T wave of 0.2 mV^0.3 mV or more in chest leads,with V3-V4 most obviously in 29 patients （76%）, and the amplitude of R wave in left chest V4-V6 increase higher, and Rv4〉 Rv5〉 Rv6 in 26 patients （68.4%）, and the ST depression in V3-V4 in 29 patients （76%）. UCG showed apical thick amount to 15 mm or above companied with ventricle narrow and small in 38 patients. Conclusion Electrocardiogram of apical hypertrophic cardiomyopathy has earlier diagnostic significance and predict value.
