老年人心功能衰竭相关胰岛素抵抗的初步临床分析 被引量：6

摘要 目的研究不同程度心功能衰竭对胰岛素抵抗的影响。方法选取33例心功能衰竭、无糖尿病史患者,按照NYHA分级分为心功能I、II级组,III、IV级组,测定空腹血糖及胰岛素水平,计算稳态模式评估法胰岛素抵抗指数(Homeostasis model assessment insulin resistance,HOMA-IR)及定量胰岛素敏感性检验指数(The quantitative insulin-sensitivity check index,QUICKI)。结果两组患者空腹血糖水平差异无统计学意义(P>0.05),随着心功能衰竭程度的加重,胰岛素分泌水平升高(P<0.01);HOMA-IR增高,QUICKI指数降低,并有统计学意义(P<0.05)。结论慢性心功能不全的患者存在胰岛素抵抗,并随着心功能衰竭的进展而加重。 Objective To evaluate the influence of different degrees of chronic heart failure on insulin resistance in elderly patients. Methods 33 non diabetic elderly patients with different degrees of chronic heart failure were divided into two groups according to the NYHA functional class. Fasting serum glucose and insulin were measured. HOMA-IR and QUICKI were calculated in two groups. Results Fasting insulin and HOMA-IR were parallel with aggravation of heart failure （P〈0.05）, and QUICKI decreased with aggravation of heart failure （P〈0.05）. Conclusions Insulin resistance is affected by chronic heart failure, and aggravates with the progression of heart failure.
