大鼠碱烧伤创面切痂植皮时机的选择

摘要 目的:通过动物实验探讨碱烧伤创面的切痂与植皮时机。方法:90只成年Wistar大鼠制作皮肤Ⅲ度烧伤模型并随机分为A、B2组,每组45只,分别于伤后1h、2h、4h、8h、12h、24h、48h、72h、96h切痂,每个时点5只。A组切痂后立即植皮,B组切痂后延迟48h植皮,测定大鼠痂下组织pH值,观察创面损伤情况以及植皮成活系数。结果:A、B组间pH值差异无统计学意义(F=2.635,P>0.05),且各时点间差异亦无统计学意义(F=0.953,P>0.05);A组术后1~8h切痂立即植皮,植皮成活系数高于12~48h切痂立即植皮,B组8h后各时点切痂延迟植皮,植皮成活系数高于8h前各时点。切痂和植皮时间存在交互效应(F交互=85.580,P<0.05)。结论:碱烧伤后1~96h,切痂时间和植皮时机对大鼠痂下组织pH值无明显影响;碱烧伤大鼠8h内切痂,立即植皮与8h后切痂延迟48h植皮植皮成活系数较高,切痂和植皮时间存在交互效应。 Aim: To investigate the opportunity of cutting scab of alkali burnt wounds and skin grafting. Methods: The Ⅲ degree alkali burn models was established on 90 Wistar rats, and allocated into group A and group B. The scabs were cut at 1 h, 2 h, 4 h, 8 h, 12 h, 24 h, 48 h, 72 h, and 96 h, then the rats in group A were performed skin graft immediately, and those in group B were performed skin graft after 48 h. The pH of the tissue under the scab was determined at the above time points and the survival coefficient of skin grafting was calculated. Results: The pH of the 2 groups and the pH among different time points had no significant differences ( F = 2. 635, P 〉 0.05 ; F = 0. 953, P 〉 0.05 ). In group A, the survival coefficient of skin grafting for rats being cut scab within 1 -8 h after alkali burn was higher; in group B, the survival coefficient of skin grafting for rats being cut scab after 8 h was higher. The time of cutting scab and grafting time had interactive effects on the survival coefficient of skin grafting (F = 85. 580,P 〈 0.05 ). Conclusion: The time of cutting scab of alkali burnt wounds and skin grafting has no obvious effects on pH. The wound of Ⅲ degree alkali burnt within 8 h should cut scab early and graft skin immediately, however, after 8 h, should cut scab early and graft skin delayed. The time of cutting scab and grafting time had interactive effects.
