缺血性脑卒中复发患者血浆纤维蛋白原检测 被引量:9

摘要 目的:探讨缺血性脑卒中复发患者血浆纤维蛋白原(Fg)的变化。方法:检测156例缺血性脑卒中复发患者(按牛津郡社区卒中计划(OCSP)分为完全前循环梗死组(TACI,17例),部分前循环梗死组(PACI,45例),后循环梗死组(POCI,28例),腔隙性梗死组(LACI,66例))、64例首次缺血性脑卒中后1~2a未复发患者及40例正常体检者血浆中的纤维蛋白原含量,并与发作类型及神经功能缺损程度进行相关分析。结果:①缺血性脑卒中复发患者纤维蛋白原含量高于正常对照组和未复发组。②TACI组纤维蛋白原含量明显高于PACI组、POCI组、LACI组(P均<0.05)。③纤维蛋白原含量与神经功能缺损程度呈正相关(r=0.89,P<0.05);Fg含量高者神经功能缺损程度评分为(27.3±7.8)分,含量低者为(17.6±5.4)分,2者相比差异有统计学意义(t=3.227,P<0.05)。结论:纤维蛋白原含量与缺血性脑卒中复发有关,并间接反应病情,提示在卒中二级预防中应予重视。 Aim : To investigate the changes of plasma fibrinogen in patients with ischemic recrudescent stroke. Methods:The plasma fibrinogen in 156 cases of ischemic recrudescent stroke, 64 cases of ischemic stroke, and 40 normal controls was tested. The ischemic stroke was divided into four subtypes ( 17 total anterior circulation infarcts(TACI) , 45 partial anterior circulation infarcts (PACI) , 28 posterior circulation infarcts (POCI) , 66 lacunar circulation infarcts (LACI) ) according to the Oxfordshire community stroke project (OCSP). Results: The content of plasma fibrinogen in ischemic recrudescent stroke group was higher than that in ischemic stroke group and controls. The content of plasma fibrinogen was higher in TACI group than those in PACI group, POCI group, and LACI group ( P 〈 0.05 ). The content of plasma fibrinogen was positively correlated with neurological functional deficit scale ( r = 0.89, P 〈 0.05 ). Conclusion : The content of plasma fibrinogen is related with recrudescence of ischemic stroke. It reflects the degree of the disease indirectly.
