胸腰段椎体骨折合并截瘫临床观察

摘要 目的观察探讨胸腰椎体骨折合并神经损伤致瘫病人的治疗时机和治疗方法。方法分析临床43例合并神经损伤的胸腰段椎体骨折病例资料。探讨该类损伤致瘫的治疗时机和治疗方式。结果本组43例均随访1~3年,椎体后突成角畸形基本矫正,压缩椎体高度恢复。术中发现硬膜囊撕裂32例,同时合并马尾神经从裂口疝脱出9例,局部碎骨片嵌压脊神经11例。急诊手术32例,术后随访一年,神经功能均有恢复,一周内手术和一周后手术神经恢复较差,按照ASLA分级标准,A级9例仅有5例恢复到B级,其余不全损伤34例均有明显改善,其中28例完全恢复。结论胸腰段椎体骨折合并神经损伤的治疗应在条件允许下急诊手术减压,越彻底越好,脊髓损伤恢复越快,效果越好。同时应用椎弓根固定器具,对骨折脱位复位效果好。固定确实,以利脊髓损伤的恢复。
