摘要
目的:探讨SE9000全血细胞分析仪在血细胞分析中对幼稚细胞的分析及形态学鉴别的临床意义及应用价值。方法:结合标本分析值并参照美国罗马琳达大学医学院临床实验室镜检标本筛选方法,把1000例标本的仪器分析结果分成4组,分别与镜检结果比较。结果:A组580例标本镜检未见异常细胞,仪器分类结果与镜检结果基本相符;B组65例标本中仪器IP信息栏提示Blast2例、Ⅰmm Grans/Left Shift35例、异常Ly8例、MRBC15例、提示2项以上5例。镜检结果28例形态异常,幼稚细胞镜检阳性率43.1%;C组102例标本中镜检发现幼稚细胞8例,幼稚细胞镜检阳性率为7.8%;D组253例标本中仪器IP信息栏提示Blast22例、I mm Grans/Left Shift117例、不典型/异常Ly28例、MRBC47例、提示1项以上39例。镜检结果201例出现形态学异常,其中幼稚细胞镜检阳性率为73.5%。结论:SE9000全血细胞分析仪IP信息栏提示存在一定程度的假阳性与假阴性;对非A组标本进行必要的镜检能避免漏检阳性结果,也能纠正仪器的假阳性指示;其对外周血中出现幼稚粒的患者在检测幼稚细胞值方面有一定的参考价值。
Objective: According to the analysis of immature cells and the discrimination of morphology in the analysis of blood cell by the SE 9000 Analysis Instrument for Classifying Blood Cells into Five Kinds, probe into the clinical significance and practical value of the check and measure of this instrument. Methods: Combine the analysis numbers of specimens consult the filtering method of microscope checks on the specimens of the clinical library in the Medical Department in Romelinda University in America, and according to whether they correspond the below conditions to divide the results of 1,000 specimens by the analysis instrument into 4 groups. Results,In group A,580 specimens do not show any abnormal cells after the check by a microscope,and the classification result of this instrument accords with the result of microscope check basically. The instrument shows the IP clues that among 65 specimens in group B, there are 2 Blast .cases, 35 Imm Grans/ left shift cases, 8 abnormal cases, and 15 MRBC cases. Besides,there are 5 cases which have more than 2 clues. On the other hand, microscope check shows 28 morphological abnormality cases.and the positive rate of immature cells reaches 43. 1% Among the 102 specimens in group C, there are 8 immature cells by handiwork microscope check, and the positive rate of immature cells is 7.8 %Among the 253 specimens in group D, the IP column clues 22 Blast cases, 117 Imm Grans/ Left Shift cases, 28 non-- typical/abnormal cases, and 47 MRBC cases. Microscope checks out 201 immature cells, and the positive rate of immature cells reaches 73.5%. Conclusions, In a way, some false positive and false negative clues exist in the IP clues of SE9000 Analysis Instrument for classifying blood cells into five kinds. If we have a microscope check on groups B,C, D, we can not only make sure the positive cases are not filtered, but also rectify the false positive clues that caused by this instrument, which provides some references to check the IMI (immature cell) of patients in whose peripheral blood there are some immature granulocytic.
