心脏机械瓣膜置换术后早期抗凝治疗中华法林对映体浓度的监测及临床意义 被引量:3

摘要 目的初步探讨心脏机械瓣膜置换术后早期抗凝治疗中华法林对映体浓度的动态变化及其临床意义。方法选择23例心脏机械瓣膜置换手术患者,分别于给药前、给药后24 h、48 h、72 h、120 h、144 h同步测定血浆华法林总浓度(W总)、华法林S-对映体浓度(WS)、华法林R-对映体浓度(WR)、国际标准化值(INR)、凝血因子Ⅶ、Ⅱ(FⅦ、FⅡ)的动态变化,同时观察临床上出血、栓塞及死亡等事件。结果23例患者均无抗凝并发症发生。首次口服华法林后24 h其WS、WR即有明显增高,并于首剂量华法林后120-144 h基本达到稳态。在抗凝治疗144 h内,WS与FⅦ、FⅡ呈负相关性(P〈0.01);WS与FⅦ、FⅡ的相关性大于W总和WR(P〈0.01)。服药后120 h内WS和W总均与INR值无相关(P〉0.05),服药后120 h后与INR值正相关(P〈0.01)。结论心脏机械瓣膜置换术后早期抗凝治疗中监测WS可较准确地反映华法林抗凝的强度。
