摘要 目的:传统的康复评估方法无法获得客观的分析数据,故通过软件化的监测方法,可在治疗过程中跟踪监测面部神经麻痹患者面部运动的变化,并建立评估档案,为医生提供诊断治疗面部神经麻痹的客观评估资料。方法:在Visual Basic6.0环境下自主开发面部神经麻痹康复治疗监测软件。可以动静态采集视频图像,根据人脸的对称性以及面部运动规律,分别在眼、嘴、舌等五官部位选取多个特征点描绘其运动轨迹,计算它们的位移量、速度、加速度以及嘴角或眼角的偏斜度等特征参数,并且依据运动过程中的参数为面部神经麻痹患者建立一个客观的数据档案。结果:记录测评结果,包括运动距离、平均速度、最大速度、平均加速度、最大加速度等特征参数。通过分析特征参数的变化趋势,辅助判断患者康复情况。软件为医生提供一个相对客观、简便的分析工具,有助于医生改进治疗方案。结论:该软件是对面部神经麻痹后面部肌肉运动功能恢复监测的创新性研究,可代替以主观评价为主的传统康复评估方法,客观地反映了面部神经麻痹患者康复情况。 AIM: Conventional rehabilitation evaluation cannot provide the objective data for analysis. In the whole therapy of facial palsy, monitoring software can periodically monitOr the facial movement change of facial paralytics and create real-time evaluation archives for assistant diagnosis of facial neural palsy. METHODS: The monitoring rehabilitation software of facial paralysis is developed independently under Visual Basic 6.0 environment. It can gather both video and photos at dynamic and static states. According to the symmetry of face and rules of facial movement, it chooses some feature points of five organs such as eyes, mouth and tongue to describe facial movement track, then conipute the feature parameters, which include moving distance, speed, acceleration and slope of angulus oris or canthus. The software also creates an objective data archives for facial paralytic by the parameters selected in the process of moving. RESULTS: The monitor software records measurement results, including moving distance, average speed, maximum speed, average acceleration, maximum acceleration and so on. The change trend of feature parameters during the process assists to diagnose the rehabilitation of facial neural palsy patients. Therefore, the monitoring software can offer an objective and simple analyzing tool for doctors to improve the treatment scheme. CONCLUSION: The monitor software is an innovation in the monitoring rehabilitation of the facial nerve function after facial neural palsy. It can replace the traditional rehabilitation evaluation methods that focus on the subjective evaluation, and reflect the rehabilitation of facial paralytics objectively.
