经皮激光盘内气化减压加臭氧溶解术联合治疗颈椎间盘突出症 被引量:1

Treatment of cervical disc herniation via decompression in the tray by percutaneous laser disc decompression and ozone dissolution

摘要 目的探讨经皮激光椎间盘气化减压加臭氧溶解术联合治疗颈椎间盘突出症的疗效。方法治疗组应用半导体激光(功率0.5~30W,波长980nm)、医用臭氧发生器,在X线引导下,选择C2~C7间隙,前路穿刺进入椎问隙中心处,导入600μm光导纤维,以自动脉冲式激光烧灼髓核(300~500J/每间隙),然后注入浓度为50μg/ml的O2-O3混合气体5~10ml。本组共治疗70例,90个椎间盘。对照组单纯应用激光气化减压术71例,92个椎间盘。结果本组病人均随访3—12个月,资料分析显示治疗组疗效优良率74.79%,总有效率92.86%,均显著优于对照组(50.70%、74,65%)(P〈0.01)。结论该联合方法治疗颈椎间盘突出症创伤小、痛苦少、安全性高,疗效优于单纯应用激光,是微创治疗颈椎间盘突出症的有效组合方法之一。 Objective To investigate the effect of decompression in the tray by percutaneous laser disc decompression and ozone dissolution on cervical disc herniation. Methods Seventy patients (90 cervical discs ) were treated with decompression in the tray by combined percutaneous laser disc decompression and ozone dissolution. Under X-ray, the C2-C7 interspaces were punctured, and 600 /.Lm ray fiber was put into the interspaces, and then the marrow core was cauterized with automatic impulse laser (300-500J per interspace). After that, intradiscal injection of 50μg/ml ozone mixture was applied. Another 71 patients(92 discs) treated by simple laser gasification decompression serverd as control group. Results The average follow-up was 3-12 months. Results showed that the outcome of treatment group was better than that of the control group ( P 〈 0.01 ). Conclusion The treatment has many advantages, such as small wound, less pain and fine security. It can serve as small invasive treatment for cervical disc herniation.
