摘要
目的分析Harrington,luque,C-D三种器械矫正脊柱侧凸的疗效。探讨术中,术后并发症的原因及防治措施。方法2000年2月~2006年10月共矫治各种原因所致的脊柱侧凸68例,平均年龄15.5岁。术前cobb’s角45°~120°,平均62°。Harrington系统矫治20例,luque(L+H)系统矫治32例,C-D系统矫治16例。结果术后平均矫正率62%,发生各种并发症16例。其中神经损伤1例,椎板椎弓骨折4例,肺部感染1例,脱钩3例,断棒2例,假关节1例,棒未端过长3例,伤口感染1例。结论根据脊柱侧凸的畸形程度,选择合适的内固定技术,把握好矫正率与安全性的尺度,减少并发症的发生。
Objective To analyse the result of three instrument (Harrington, Luque and C -D) during correction of scoliosis. To evaluate the cause of the complications and dicuss how to prevent and manage them. Methods Sixty - eight cases of scoliosis were corrected at an average of 15.5 years from February 1992 to Octoberl998. The average preoperative Cobb angle was 62° (range 45°~ 120°). 20 patients were treated with the Harrington instrumeatation, 32 patients were treated with the Luque instrumentation, 16 patients were treated with the C -D system. Results The average rate of correction was 62% ,meanwhile complications were found in 16 cases. There were intraoperative laminar or pedicle fractures in 4, neurological injury in 1, pneumonia in 1 ,loosening in 3, rod breakage in 2 ,pseudojoint in 1 ,bulge of C - D rods in 3 ,and infection in 1. Conclusion The 3 kinds instrumentation should be used base on the degree of the scoliosis, it is very important to balance the ratio of the correction and the safety to decrease the complications.
