孕晚期孕妇抑郁情绪对分娩的影响及对策

目的了解孕晚期(35周至分娩前)孕妇抑郁发生情况及对分娩过程的影响。方法采用汉密顿抑郁量表(HAMD)对340例孕晚期孕妇进行抑郁状况调查,记录发生抑郁与未发生抑郁孕妇分娩各产程时间及产后出血量,并进行统计学比较分析。结果无抑郁135例(39.7%),有抑郁205例(60.3%),其中严重抑郁6例(2.9%),中度抑郁53例(25.9%),轻度抑郁146例(71.2%);抑郁组产妇第一、第二产程较无抑郁组延长,而且出血量增多。结论孕晚期孕妇中抑郁情绪普遍存在,而且抑郁情绪与产程及产后出血量密切相关;做好产前孕妇的心理健康工作,给予科学的产前指导,可帮助孕妇顺利度过妊娠期、分娩期及产育健康的后代。

Objective To evaluate influence of depression of the late pregnancy on delivery. Methods HAMD was used to evaluate depression in 340 pregnant women before delivery. The volumes of hemorrhage were recorded at every stage of labour for 340 pregnant women and compared between those with hemorrhage and those without. Results 135 （39.7%） pregnant women had no depression and 205 （60. 3% ） had. Among 205 pregnant women who had depression, 6 （2. 9% ） had severe depression, 53 （25.9%） middle depression, 146 （71.2%） mild depression. The first and second stage of labor was significantly prolonged and the volume of hemorrhage was significanlty larger in those with depression, compared to those with depression. Conclusion Depression occurs to the pregnant women at medium and late stage of pregnancy. Depression is closely related to labor and the volume of postpartum hemorhage. Therefore, prepartum mental care and scientific instruction before delivery may help pregnant women through gestation and delivering stage.
