老年外阴癌广泛切除术的护理

目的总结外阴癌切除术的护理方法。方法2004年7月～2007年7月对7例外阴癌老年患者进行手术治疗，并配合做好术前后的护理。结果患者术后均无并发尿路感染、皮瓣坏死、下肢水肿、压疮等并发症，伤口愈合良好，住院天数20～30d，平均25d。结论由于老年人抵抗力低，而且手术范围广泛、创面较大，术后卧床时间长，易发生术后伤口感染、循环障碍等并发症。因此，术前应加强患者心理疏导，做好预防术后感染的术前准备；术后密切观察患者病情的变化，做好创口及会阴部护理，保持引流通畅，积极预防下肢循环障碍、下肢血栓性静脉炎等的发生。

Objective To explore the nursing methods of vulvar carcinectomy. Method From July of 2004 to July of 2007, 7 elderly patients with vulvar cancer underwent radical surgery and peri-operative nursing care. Results No patients experienced genitourinary infection, edema of lower extremities, and bedsore. The hospitalization lasted from 20 to 30 days, with a mean of 25 days. Conclusions Elderly patients with vulvar cancer are usually feeble. Moreover, the operation is extensive and aggressive so that the hospitalization time is usually very long. The patients are prone to some complications such as cut infection and circulation problem. Therefore, it＇s important to give psychological advice and take some procedures to prevent postoperative infection before operation. After the operation, it is critical to closely observe the patients, conduct special nursing care to the cut and perianal area, keeping drainage unobstructed, active prevention of lower extremity obstruction and thrombophebitis .
