脉压对冠心病患者左室肥厚和心功能的影响

Effection of Pressure Difference on LV Pachynsis and Heart Function of CHD

目的观察脉压对冠心病患者心脏功能及左心室大小的影响。方法冠状动脉造影确诊的110例冠心病患者,排除合并有原发性或继发性高血压,分别测定每例患者的24 h的动态血压。根据24 h平均脉压分为3组,A组:脉压≤5.98 kPa(45 mmHg),B组:7.32 kPa(55 mmHg)≥脉压>5.98 kPa(45 mmHg),C组:脉压>7.32 kPa(55 mmHg),比较3组的24 h平均动脉压,心电图中Rv5+Sv1,心脏彩色超声左心室质量指数(LVMI),左室后壁厚度(LVPWT)及射血分数(EF)。结果脉压明显增高的C组较A组及B组LVMI、LVPWT明显增加,Rv5+Sv1也明显增加,但EF下降,差异有统计学意义。结论脉压增高使患者的左心室肥厚更加明显,心功能明显下降,控制脉压有助于心功能的改善。

Objective To observe the effection of preesure difference on LV pachynsis and heart function of CHD. Methods 110 cases of CHD patient were. detected with coronary angiography and each primary and secondary hypertension patient was detected Bp 24 h. And they were divided into 3 groups： pressure difference （PD） less than 45mmHg in group A, PD was between 45-55 mmHg in group B and PD was more than 55 mmHg in group C. The PD, ECG, Doppler, LVMI, LVPWT and EF were compared. Results The LVMI, LVPWT and Rv5 ＋ Svl in ECG were increased in group C but there was significant difference in EF comparing with group A and B. Condusion The high PD might increase LV pachynsis and decrease heart function. Controlling PD might be benefit for improvement of heart function.
