抗CCP和抗RA33抗体检测在类风湿性关节炎中的应用

摘要 目的探讨环瓜氨酸多肽(CCP)抗体和RA33抗体对类风湿性关节炎(RA)的临床应用价值。方法47例RA患者、25例未分化关节炎患者、29例其他自身免疫病患者分别采用ELISA法检测CCP抗体、RA33抗体,速率散射比浊法检测CRP(C反应蛋白)、类风湿因子(RF)、抗链球菌溶血素O(ASO),魏氏法检测红细胞沉降率(ESR)观察各指标对RA的诊断及治疗价值。结果RA患者抗CCP抗体、抗RA33抗体以及RF的敏感性分别为63.8%,44.7%和93.6%,特异性为93.1%,89.6%和68.9%;两种抗体对未分化关节炎诊断为RA的确诊率为63.6%,55.6%。CRP与ESR有良好的相关性,与ASO一起可作为RA患者病情监控的指标。结论CCP抗体、RA33抗体有助于RA的早期诊断特异性,而各项指标的联合检测有助于对RA早期诊断、治疗、病情预后的估计。 Objective To explore the diagnostic value of anti cyclic citrullinated peptide antibody( anti CCP) and anti RA33 antibody in patients with rheumatoid arthritis. Methods The anti CCP and anti RA33 antibody were detected by ELISA , C reaction protein (CRP) rheumatoid factors (RF) Antistreptolysin O (ASO) determined by latex agglutination test Westergren' method to Measure Erythrocyte sedimentation rate(ESR), respectively, in 47 RA ,29 other rheumatic diseases and 25 undifferentianted arthritis . Results The sensitivities of anti CCP and anti RA33 antibody , rheumatoid factors (RF) were 63.8% ,44.7% and 93.6% ;the speeificities were 93.1% ,89, 6% and 68.9% ;, respectively, there are good close relationship between anti CCP and anti RA33 antibody ASO are important to be in radiographic stage and patients assessment of illness. Conclusion Anti CCP and anti RA33 antibody are helpful in the early diagnosis of. RA. Combined detection of the above four antibodies have a better discrimination ability which are complementary with each other, upgrade the detectable rate and have significat clinical value in diagnostic classification and prognosis and treatment of RA.
