B型超声检查诊断玻璃体后脱离的应用

Evaluation of type-B ultrasonic in diagnosing the posterior vitreous detachment

目的探讨B型超声检查诊断玻璃体后脱离(PVD)的临床意义。方法根据术前B型超声检查结果诊断为PVD者26例和非PVD44例,玻璃体切割术中应用曲安耐德(TA)辅助玻璃体后皮质可视化,以术中所见的PVD为诊断标准,分析评估B型超声诊断PVD的价值。结果玻璃体切割术中TA辅助玻璃体后皮质可视化发现的PVD29例,非PVD41例,B型超声诊断PVD的符合率为87.14%,分型符合率为71.42%。结论B型超声检查是玻璃体后脱离的重要诊断方法,病情的多变性和黄斑区特殊结构是其诊断失误的重要原因。

Objective In recent years,B ultrasound has been widely used in ophthalmological field,and it is playing an important role in the diagnosis of posterior vitreous detachment （PVD）. The aim of this research was to investigate the clinical value of B ultrasonic in diagnosing PVD,and verify the results during the vitrectomy. Methods Vitrectomy was performed in 70 eyes of 70 patients during the July 2004 to February 2006. Triamcinolone acetonide was used in operation to visualize the posterior vitreous cortex. Before the operation, B ultrasonic examination was taken on the eyes. The outcome of B ultrasound examination was compared with interpretative statues. Results Among the 70 eyes,26 eyes were PVD,and 41 eyes were non- PVD before operation under the B ultrosonography. The diagnosis coincidence between B ultrosonography and triamcinolone acetonide visualized in operation was 87.14% , and their classification coincidence was 71.42%. Conclusion Type-B ultrasonography is an important method in diagnosing PVD,and the diversity of vitreous-retina disease and the special structure of macular region are the main reasons of misdiagnosis for PVD.
