摘要
目的:探讨氯氮平诱发白细胞减少与细胞因子的关系。方法:监测采用氯氮平治疗的精神分裂症住院患者的全血细胞计数,选取服药后发生白细胞减少的患者作为研究对象,在患者发生白细胞减少时以及恢复正常后分别采样,采用酶联免疫吸附法检测两次的血浆IL-2、IL-6的水平,并以30例健康志愿者作对照。结果:1.发生白细胞减少时研究组的IL-2水平显著高于对照组(P(0.05),IL-6水平显著低于对照组(P(0.05)。2.研究组白细胞计数恢复正常后较白细胞减少时有显著性升高(P(0.05),白细胞计数恢复正常后的IL-2、IL-6水平与白细胞减少时比较均有显著性升高(P均(0.05)。3.白细胞计数与IL-2水平呈正相关(P(0.05),白细胞计数与IL-6水平呈正相关(P(0.05)。结论:提示氯氮平诱发白细胞减少,可能是氯氮平在治疗精神病的过程中,引起IL-2、IL-6水平的降低,导致造血系统调节功能的紊乱,从而引起白细胞减少。
Objective: To explore the relationship of clozapine - included leucopenia and cytokines. Methods: Monitoring complete blood count of first - episode schizophrenic clozapine - induced in hospital, select the patients leucopenia post - treatment to the study group, survey the levels of Interleukin - 2 ( IL - 2 ) and Interleukin - 6 ( IL - 6 ) in plasma while leucopenia and after normal with ELISA, and 30 healthy volunteers as controls. Results. 1. the IL - 2 level of study group was significantly higher than control group ( P 〈 0.05 ) , the IL - 6 level of study group was significantly lower than control group ( P 〈 0.05 ). 2. In the study group, the second count of leucocyte was significantly higher than the first( P 〈 0.05 ) , the second IL -2 and IL -6 level after count of leucocyte normal was significantly higher than which while leucopenia( P 〈 0.05 ). 3. There was significantly correlation between the count of leucocyte and the IL- 2, IL- 6 level in the study group( P 〈 0.05 ). Conclusions: The study indicates the likely mechanism of leucopenia in clozapine treatment. In the course of treating the patients of schizophrenia with clozapine, clozapine change the levels of cytokines, cause the turbulence of accommodate function of blood system, thereby, the leucocyte decreased.
