摘要
目的探讨隐蔽切口内镜辅助淋巴管瘤内贯通烧灼术加术后负压吸引治疗淋巴管瘤的可行性、安全性及治疗效果。方法从2004年2月至2006年1月,我们采用隐蔽切口内镜辅助淋巴管瘤内贯通烧灼术加术后负压吸引,治疗18例淋巴管瘤。术前选择的标准是:巨大海绵样淋巴管瘤,最小横径大于4cm,未行注射治疗的淋巴管瘤患儿。通过在隐蔽皮肤处做一个约3cm的切口,在内窥镜电视显示和肉眼观察下,不用充气,皮外牵拉暴露瘤腔进行手术。结果18例患儿术后均行负压吸引治疗。有两例术后注射平阳霉素两次痊愈。术后随访两年未见复发。结论内窥镜辅助淋巴管瘤内贯通烧灼术加负压吸引术后平阳霉素注射治疗淋巴管瘤是可行和安全的,创伤小,美容效果较好。
Objective To explore the clinical effects of endoscope cautery with negative pressure in the treatment of lymphangioma. Methods 18 cases of lymphangioma were treated with this method from Feb. 2004 - Jan. 2006. The indication was giant spongiform lymphangioma and the diameter of focus was more than 4cm without injective therapy before. Results All patients demonstrated the satisfactory improvement with 2 years follow up. No recurrence was found. Conclusion The method of endoscope cautery with negative pressure is reasonable, simple and effective for treatment of lymphangioma.
