摘要
目的:颅脑肿瘤注入Gd-DTPA(磁显葡胺)以区别瘤体与水肿,鉴别肿瘤良、恶性及囊壁,显示脑转移瘤和子瘤灶,达到临床诊断目的;资料和方法:72例颅脑肿瘤,其中脑内肿瘤48例,脑外肿瘤24例。均用ASM-015P永磁型MRI扫描机,采用头线圈,SE序列:T1加权像TR=500ms,TE=30ms,T2加权像TR=2000-3000ms,TE=100-120ms,,层厚5-10mm,方位行横、冠、矢状位,增强扫描为:静脉缓慢注入Gd-DTPA(磁显葡胺)按01-02ml/kg体重,注药后5、10、25、35min作相应方位T1加权像扫描;结果:1脑内肿瘤48例,胶质瘤33例,明显及部分强化25例,不强化8例,脑转移15例均强化共39个瘤灶(其中新发现8个);(2)脑外肿瘤24例,脑膜瘤14例,垂体瘤6例,听神经瘤3例上述均强化,硬膜外表皮样囊肿1例不强化;结论:颅脑肿瘤通过静脉注入Gd-DTPA(磁显葡胺)可以明确瘤灶与水肿及发现T2加权像上被水肿掩盖的子灶,达到临床诊断目的。
PURPOSE: By means of venous injection of Gd-DTPA, benign and malignant cranioverebral tumors as well as their walls can be differenciated; and the cerebral metastases of tumors and seeding focus can be showed in order to obtain the clinical diagnoses;MATERIAL ANP METHDD: 72 cases of craniocerebral tumors were diagnosed including intracerebral tumors (48 cases),extracerebral tumors(24 cases),which have been identified by means of operation, pathology of clinical follow-up. Permenant Magnetic Model MRI scanner (0 15T ASM-O15P)was used with head coil, SE Senes:T 1 weighted image TR=500ms,TE=30ms, T 2 weighted image TR=2000-3000ms TE=100-120ms, slice thickness 5-10mm, Transverse, Coronal and Sagittal location. Gd-DTPA(0 1-0 2ml/per kg body weight)was slowly injccted intravenously to intensify MRI scanning. The relative position scaning were down 5,10, 25,35min post-injection; RESULTS: (1)Of the 48 cases of intacerebral tumor ,33 cases were gliomas, the apparent and partial intensified were 25 cases, no intensefied 8 cases, Cerebral metastasis of tumors intensified completely 15 cases. There are 39 tumors all together(among them, 8 cases were newly fourded)(2)Of the 24 cases Extracerebral tumors:including 14 cases of meningioma, 6 cases of pituitary tumor, 3 cases of Acoustic neuroma. All of them were intensified. Only the one case of epidural epidermoid cyst was not intensified;CONCLUSION: Throngh injection intavenously with Gd-DTPA, the focus, oedema and seeding focus, which was covered by oedema in T 2weighted image, could be discovered by MRI scanning, So the diagnosis can be made.
