改良式PPH与PPH治疗Ⅲ～Ⅳ期痔的对比研究

Contrastive study of treatment for Ⅲ～Ⅳ degree hemorrhoid between improved PPH and PPH

目的探讨改良式吻合器痔上黏膜环切术（PPH）治疗Ⅲ～Ⅳ期痔的临床价值。方法160例Ⅲ～Ⅳ期痔患者随机分为改良式PPH组（82例）与PPH组（78例），比较两组的治疗效果以及并发症情况。结果两组在术中出血、术后出血、术后疼痛、尿潴留、肛缘水肿、住院时间以及肛门功能方面差异无统计学意义（P〉0．05）；改良式PPH组手术时间（14．2±2．4）min，复发率为0，而PPH组分别为（18．4±2．3）min、7．7％，两组比较差异有统计学意义（P〈0．05）。结论改良式PPH具有手术时间短、操作方便、复发率低等优点。

Objective To investigate the chnical value of procedure for prolapse and hemorrhoids （PPH）which had been improved in the treatment of Ⅲ～Ⅳ degree hemorrhoid. Methods One hundred and sixty patients with Ⅲ～Ⅳ degree hemorrhoid were divided into two groups randomly. Eighty-two cases were treated with improved PPH （improved PPH group）and 78 cases were treated with PPH （PPH group）. And compared the therapy and comphcation condition between the two groups. Results There were no significant statistical differences including bleeding in and after operation ,pain after operation, retention of urine, edema around anus,hospitalization time and the function of anus between the two groups （P 〉 0.05）. The average time of operation were （14,2±2.4） minutes and （18.4 ± 2.3 ） minutes in improved PPH group and PPH gruop, the rate of relapse were 0,7.7%, respectively, they were statistically different significantly （P 〈 0.05 ）. Conclusion Improved PPH is simple and convenient with shorter term of operation and fewer complications.
