208例颅脑损伤患者并发上消化道出血的治疗

Treatment of Upper Gastrointestinal Hemorrhage Complicated of Head Injury:Report of 208 Cases

目的总结颅脑损伤后上消化道出血的预防措施及其治疗方法。方法回顾性分析208例颅脑损伤后上消化道出血的病例,在治疗原发病基础上,采用止血、制酸、保护胃黏膜等对症处理,胃镜下治疗8例;胃次全切除术1例。结果治愈172例,缓解15例,无效3例,死亡10例,胃镜下治愈8例。结论预防和治疗的重点是早期保护胃黏膜,早期发现上消化道出血,早期治疗。必要时应及时使用特殊治疗手段,以迅速控制出血病情。

Objective To investigate the prevention and treatment of upper gastrointestinal hemorrhage complicated of head injury. Methods Two hundred and eight cases of head injury complicating upper gastrointestinal hemorrhage were retrospectively reviewed. Based on managing primary injury,hemostasis,inhibiting acid and protecting mucus were adopted, 8 were treated with focal hemostasis under endoscopy and 1 received partial gastroectomy. Results 172 cases cured, 15 were ease, 3 were in vain, 10 died,8 cases cured with stomach mirror. Conclusion The emphasis of prevention and treatment focused on early period to protect gastric mucosa, early period to discover upper gastrointestinal hemorrhage and early treatment. Special procedure should be carried on in time in order to control the condition if necessary.
