目的:探讨新医学模式下如何建立和完善糖尿病患者随访档案。方法:通过分析糖尿病患者随访的现状,提出通过建立糖尿病患者随访档案,对糖尿病患者进行积极、负责的随访,可以及时、定期了解病情的变化过程,然后根据病情变化,科学指导糖尿病患者定期复查、复诊和及时进行治疗。结果与结论:通过有效干预,提高了我院随访糖尿病患者的治疗效果,提升了他们的生命质量。

Objective. To set up and manage the periodic follow - up record for diabetics under the new medical mode. Method： The paper studied the status of the management of follow - up for diabetics, and points out that through active and responsible foUow - up of diabetics, doctors can timely and periodically e- valuate the disease development and instruct the patients to have periodically re - examine, re - diagnose and further treatment.
