≥65岁老年心力衰竭59例的临床特点及治疗

Clinical features and treatment of 59 elderly patients aged ≥ 65 years with heart failure

目的:分析≥65岁老年心力衰竭的临床特点,为诊治老年人心力衰竭提供依据.方法:将心力衰竭患者112例按年龄分为≥65岁组和<65岁组,分别对其临床特点进行对比分析.结果:随着年龄的增加心力衰竭有其特点,≥65岁的老年患者多为冠心病(50.8%),其次为肺心病(20.3%);临床症状不典型,程度以心功能III和Ⅳ级为多,高龄死亡率较高(87.5%).结论:≥65心力衰竭患者主要病因为冠心病,年龄越大,临床症状越不典型,心力衰竭程度越重,预后不良;治疗上要区别对待.

AIM： To analyze the clinical features of elderly patients of 65 years old or more with heart failure and to provide some evidence for the treatment of heart failure in elderly patients. METHODS： 112 cases with heart failure were divided according to their age into ≥65 years group （n =59） and 〈65 years group （ n = 53 ）. Their clinical features were compared and analyzed statistically. RESULTS： Clinical manifestations of heart failure differed very much with the age increasing. Pathogenesis of heart failure for old people aged ≥ 65 years mostly was coronary heart disease （ 50.8% ）, secondly was pulmonary heart disease （20.3%） ; compared with 〈 65 years group, clinical symptoms were less typical, and heart function Ⅲ and Ⅳ were more frequent （49.1% , 20.3% ） , and the mortality was higher in the ≥ 65 years group（87.5% ）. CONCLUSION： The most important cause for heart failure in .≥65-year-old patients is coronary heart disease. With the age getting older, the clinical symptoms are less typical, the degree is more serious and the prognosis is less satisfactory. Different treatment should be given accordingly for various aged patients with heart failure.
