Diagnosis and Treatment of Esthesioneuroblastoma in Nasal Cavity and Sinuses

【目的】探讨鼻腔鼻窦嗅神经母细胞瘤的临床特点及诊断治疗方法。【方法】回顾性分析本科2003年1月至2007年4月手术切除21例嗅神经母细胞瘤的临床资料,对其临床表现、诊断和手术径路、预后进行探讨。【结果】本组单纯手术6例,2例术后1年内复发,手术+放疗及化疗15例,2例1年内复发。复发病例经再次手术加放疗及化疗,随访1年3例无复发,1例带瘤生存。【结论】嗅神经母细胞瘤早期诊断困难。行鼻内窥镜下经额鼻联合径路切除是较理想的手术方式。术后联合放疗及化疗十分重要。

To discuss the clinical characteristics, diagnosis and treatment of esthesioneuroblastoma（ENB）. [Methods]The clinical date of 21 patients with ENB were analyzed retrospectively, and discussed its clinical characteristics, diagnosis, surgical approaches and prognosis. [Results]Six cases treated by surgery alone, and two cases recurred in a year. Fifteen cases treated with surgery combined with radiotherapy and chemotherapy, and also two cases recurred in a year. All recurred cases received re-operation combined with radiotherapy, three cases survived without tumor and one survived with tumor after one year follow-up. [Conclusion]It is difficult to diagnose ENB early. Trans-nasal-frontal approach with nasal endoscope is effective approach. Postoperative radiotherapy and chemotheranv are very important to the prognosis of ENB.
