重型颅脑损伤头CT图像计分与颅内压及预后的关系

Relationship of CT score with intracranial pressure and prognosis in severe head injury

目的明确头CT图像计分与重型颅脑损伤患者颅内压及其预后的关系。方法重型颅脑损伤患者96例,于入院时即行颅内压监测,并于伤后24小时行头CT平扫,测量CT图像并计分,于伤后3个月对每例患者行格拉斯哥预后评分(GOS),将CT图像计分分别与ICP数值、GOS对照、比较,以进一步明确头CT图像计分与颅内压及预后的关系。结果头CT图像计分越高,颅内压升高越明显,患者预后越差,生存质量越低。结论头CT图像计分能较好地估计颅内压,并可指导治疗,判断预后。

Objective To observe the relationship of CT score with intracranial pressure and prognosis in severe head injury. Methods 96 patients with severe head injury were selected for this study. All the patients were under ICP monitoring when they came to hospital, and had head CT scanning within 24 hours since they were injured and every patient was given a GOS mark 3 months later. The relationship of CT score with intraemaial pressure and GOS were estimated. Results A significant correlation was found betwecn CT score and ICP. The higher CT score there are the higher ICP arm the poor prognosis correlated with（ P 〈 0.05）. Coneluslon CT score could estimated the ICP and the prognosis in severe head iniury.
