膈神经阻滞联合内关穴注射预防胆囊手术牵拉反应的临床观察

Diaphragm nerve block combined with point injection in Neiguan alleviates the stretch reaction in gallbladder surgery

目的观察膈神经阻滞联合内关穴注射哌替啶预防胆囊手术牵拉反应的效果。方法80例胆囊手术患者随机分为4组(每组20例):A组采用膈神经阻滞联合内关穴注射哌替啶;B组内关穴注射哌替啶;C组术中静脉注射哌替啶;D组术中不用辅助药。结果A组减轻胆囊手术牵拉反应效果明显优于B、C、D 3组。结论膈神经阻滞联合内关穴注射哌替啶是减轻胆囊手术牵拉反应的较好方法。

Objective To observe the effect of diaphragm nerve block combined with injection of meperidine in Neiguan point in relieving the stretch reaction during the course of gallbladder operation. Methods 80 patients undergoing gallbladder surgery were randomly divided into 4 groups （ n = 20 each） ： group A was prescribed diaphragm nerve block combined with meperidine injection at Neiguan point; group B, only meperidine injection at Neiguan; group C, meperidine iv. ; group D, no adjuvant medication. Results The alleviative effects on stretch reactions were evidently better in group A than in groups B, C and D （P 〈 0.01 ）. Conclusion Diaphragm nerve block combined with point injection could be a good method for the relief of stretch reactions during the course of gallbladder operation.
