老年人腹腔镜胆囊切除术围手术期的处理

The perioperative perild managements of laparascopic cholecystectomy in aged patients

目的:探讨老年患者腹腔镜胆囊切除术(LC)的可行性及围手术期的处理。方法:比较、分析老年胆囊炎LC组及开腹胆囊切除术(OC)组的临床资料。结果:平均手术时间、术后腹腔引流量两组比较差异无统计学意义(均P>0.05)。而OC组平均术后住院天数、自主下床活动时间、肠功能恢复时间较LC组显著延长,差异有统计学意义(P<0.01)。结论:对于老年胆囊炎患者术前充分准备,术中术后加强监测以及把握手术技巧,行LC安全可行。

Objective： To explore the feasibility and perioperative period managements of laparoscopic cholecystectomy （LC）. Methods： The clinical data of aged patients with cholecystitis treated by laparoscopic cholecystectomy were retrospectively analyzed and compared with that of patients treated by open cholecystectomy （OC）. Results： There was no significant different in the mean time and the quantity of flowing liquid from abdominal cavity after operation between the two groups （P〉0.05）. But the patients treated by OC had longer postoperative recovery, resumed normal intestinal peristalsis slower and were discharged from hospital later, which had significant stastical difference （P〈0.01）. Conclusion： LC is safe and feasible for aged patients with cholecystitis if adequate preoperative preparations, intersive observation and care in and after operation, command of skilled operative techniques are well performed.
