成人肾母细胞瘤1例报告并文献复习

Adult Wilms Tumor:A Case Report and Literature Review

目的:提高对成人肾母细胞瘤(AWT)的认识及诊治水平,降低误诊率。方法:通过1例AWT报告,结合文献复习,针对AWT的病因、诊断和治疗等特点尤其是中国AWT现状进行综合分析。结果:由于AWT临床罕见、诊治经验不足,本例患者术前误诊,术后治疗不规范,肿瘤短期内复发。结论:AWT病因不明,可能与WT1基因突变有关。AWT易误诊为肾癌,早期、正确的诊断应综合各方面资料尤其是影像学及病理学检查。治疗上应尽可能手术切除肿瘤,术后根据病理分期进行辅助性化疗和(或)放疗。

Objective：To enhance the cognition and elevate the diagnosis and treatment level of adult wilms tumor （AWT）.Methods：One case of AWT was reported with reviewing the literature.Comprehensive analysis was made on the characteristics about etiology,diagnosis and treatment of AWT, especially the current domestic situation.Results：The reported patient was misdiagnosed before operation because of rare occurrence and poor experience of AWT.The treatment wasn＇t standard and the tumor recurred rapidly after operation.Conclusions：Etiological factors of AWT are still indefinite and one of which may be related to WT1 genetic mutation.AWT is easily misdiagnosed to be renal carcinoma.Early and exact diagnosis should be made by comprehensive analysis on all-round data especially the imageology and pathology.The tumor should be resected as far as possible and adjunctive chemotherapy and/or radiotherapy are necessary according to pathologic staging.
