双腔频率适应性起搏对正常QRS患者房颤和心力衰竭的远期影响 被引量:2

摘要 目的观察正常QRS患者在长期频率适应性起搏时是否会导致右室起搏比例增多进而引起房颤增加和心功能损害。方法163例行双腔起搏器治疗的患者被随机分为DDD起搏组及DDDR起搏组;DDDR组再分为3个亚组:右室起搏(VP)组;经房室结传导组;部分右室起搏组。所有患者随访2年,比较各组心力衰竭和房颤的发生率。结果亚组分析显示在部分右室起搏组中的心力衰竭、房颤发生率及VP%于DDDR组较高(P<0.05)。结论双腔频率适应性起搏可能会在部分患者中通过右室起搏比例增多而损害心功能,对双腔频率适应性起搏患者通过各种方式减少右室起搏比例对防范此种不良趋势有重要意义。
